ヨガレッスン同意書

By participating in Link & Learn International yoga lessons, I acknowledge and agree to the following terms.

  • I will follow the guidance of the instructor and assume all risks associated with the practice, taking care to prioritize my safety and stay within my physical limits.
  • If I have any pre-existing medical conditions, such as allergies, injuries, illnesses, pregnancy, or physical impairments, I will consult with my doctor and obtain their consent before attending the class.
  • If there are any first aid measures that I would like the instructor to be aware of in case of an emergency, I will declare them before participating.
  • If any abnormalities in my physical condition occur during the lesson, I agree to immediately suspend participation and report it to the instructor.
  • I agree to take responsibility for any injuries, illnesses, subsequent complications, death, or miscarriage that may occur during the lesson, and I also agree not to hold the parties involved liable regardless of the cause.
  • I agree to take responsibility for the loss or theft of personal belongings.

Link & Learn Internationalが開催するヨガレッスン受講に際し、下記内容について同意・遵守いたします。

  • インストラクター指導の下、レッスンを行い、私個人の責任の下に、安全に注意を払い、自らの身体的限界を超えない範囲でレッスンに参加することを承諾します。
  • （既にアレルギー体質の方、負傷中の方、疾病中の方、妊娠中の方、先天的あるいは後天的な理由で、身体機能の一部に障害をお持ちの方は）主治医に相談し、且つ、主治医の承諾の下、クラスに参加します。
  • 前項に該当される方で、緊急の際にインストラクターに知っておいてほしい応急処置がある場合は、参加前に申告します。
  • レッスン中に体調などに異常が生じた場合は、直ちに参加を中断し、インストラクターに報告することを承諾します。
  • レッスン中に負傷・疾病などが発生し、罹病した場合、後遺症が発生した場合、死亡した、流産した場合についても、自ら責任を負うことを承諾し、その原因の如何に関わらず関係者に対する一切の責任を問わないことを承諾します。
  • 私物の紛失や盗難などの責任は自ら負うことを承諾します。
